How to make a young carer referral
To receive the young carers service, young people need to be registered with us.
Anyone can make a referral to the young carers service, professionals, parents or young carers themselves.
The young person you are referring must be between 5 and 18 years old and they must live in York. They must be providing care for someone in their family who has an illness, long term health problem, disability, mental health problem or be misusing drugs or alcohol.
How to refer a young carer
You can make a referral by completing the form at the bottom of the page. You can also call the young carer team on 01904 715 490 to discuss a situation or make the referral over the phone, we know there is sometimes more to say then you can fit on the form.
Once we receive the form we will contact the family and arrange a home visit. At the visit we will complete an assessment of the caring role to ensure that we are the right service for this young person and their family. We will also explain all about the service, what we do and how everything works.
If you are a school or health professional is really important that you talk to the young carer and their family before making a referral. Some people are nervous about the ‘label’ young carer but we are always happy to discuss any apprehensions over the phone or at the home visit.
