How to make a young carer referral

To receive support from us, young people need to be registered with our service.

Anyone can make a referral to the young carers service – professionals, parents or young people themselves.

The young person must be between 5 and 18 years old and they must live in York. They must be providing care for someone in their family who has an illness, long term health problem, disability, mental health problem or be misusing drugs or alcohol.

Once we receive the form we will contact the family and arrange a home visit. At the visit we will complete an assessment of the young person’s caring role to ensure that we are the right service for the young person and their family. We will also explain all about what we do and how everything works.

If you are a school or health professional  is really important that you talk to the young carer and their family before making a referral. Some people are nervous about the ‘label’ young carer; we are always happy to discuss apprehensions around this over the phone or at the home visit.
