A booklet has been designed to help you to think about your own health and caring needs. The questions are for you to complete whenever you have a spare minute to yourself . You don’t need to do it all in one go – you can always go back and complete another section at another time.

Registration form

If a carer wants to register with York Carers Centre (so that they can use our services and receive our discount card and newsletters) they need to complete this registration form. Carers also need to read our Date Protection Policy. Please don't forget to get the carer to sign the form and return it to us by post at 17 Priory Street, York. YO1 6ET.

We've just got a new York Carers Centre leaflet. It briefly explains what a carers is and the sort of help we offer unpaid carers. We will try to get this leaflet into as many places as possible in York so that we can reach carers who might not have heard of us before.
